On Point 4 Veterans (OP4V)
Thank you for taking the time to learn more about us. My name is Mike Coonan, LMSW, ACSW, BCD. I am the founder of On Point 4 Veterans, a combat veteran wounded in action, and for more than 15 years, I have worked exclusively with veterans through the VA system and in private practice.
I specialize in comprehensive Military History and Psychosocial Assessments (MH-PSA) for veterans with service-connected PTSD, MST, Depression, Anxiety, insomnia, and other mental health conditions.
I bring both clinical expertise and lived military experience to this work.
What We Provide
We provide a thoughtful, trauma-informed clinical evaluation designed to fully understand you, not just your symptoms, but your entire story.
This is not a one-time review.
This is a process we go through together.
Our work includes:
• A full lifespan psychosocial history (childhood through post-service life)
• Detailed military service and trauma review (combat, MST, and non-combat stressors)
• Functional impairment analysis (work, relationships, sleep, anger, daily living)
• DSM-5–informed diagnostic formulation
• A clear narrative of occupational and social impairment relevant to VA standards
• Spouse or significant other interviews when appropriate
You will have multiple direct conversations with me, and I will stay with you through the process, from initial contact through completion of your MH-PSA.
This is comprehensive clinical work grounded in accuracy, clarity, and respect.
What We Do Not Provide
We are not a Veterans Service Organization.
We do not submit claims.
We do not provide legal representation.
We do not guarantee outcomes. We do not do therapy.
You remain fully responsible for and in control of your claim.
Our role is clinical evaluation, and documentation is thorough and correct.
What Makes This Different
Many services provide a single record review or a stand-alone document. We take a different approach.
You are working directly with a clinician who:
• Takes the time to understand your history in full
• Helps organize and clarify your experience
• Identifies what is missing and what matters
• Stays engaged with you throughout the process
This is not a transaction. It is a structured clinical process.
How You Can Work With Us
Option 1: Do It Yourself (Free)
Use our website and podcast to organize and describe your own service-connected mental health history.
You can complete this process at no cost and submit your own claim.
Option 2: Work Directly with Me - $600 Flat Fee
This includes:
• Multiple structured discussions with you
• Spouse/significant other interview when appropriate
• Record review
• Clinical evaluation of your condition
• Development of your comprehensive MH-PSA
There are no hourly fees.
There are no increasing costs for complexity.
There are no outcome-based charges.
A Note About Cost
Some online services charge thousands of dollars for a single document. Others use models where fees are tied to a portion of your future disability benefits over time.
You should understand your options and make the decision that is right for you. Our goal is to provide thorough, clinically grounded support at a reasonable, transparent cost.
Why This Matters
Many veterans struggle to explain the full impact of trauma on their lives. Symptoms are minimized. History is incomplete. The connection is not clearly shown.
A structured, professionally developed psychosocial assessment helps ensure:
• Your history is complete
• Your condition is clearly understood
• Your functional limitations are accurately documented
You earned a thorough and respectful evaluation. You deserve to have your story presented clearly and taken seriously.
