Guided Military History by you, the veteran, called (MH-PSA), alongside our full Military History Psychosocial Assessment called (MHPSA)

This system has been developed by On Point 4 Veterans (OP4V) to serve more veterans professionally and economically, to address the real financial, policy, and practice barriers veterans encounter when seeking recognition and care for service-connected mental health conditions. Our guided MH-PSA evaluation process is designed to restore fairness, accuracy, and dignity to a VA Disability Compensation evaluation process that too often confuses or discourages veterans it was meant to serve.

Clinical Purpose of our Assessment System

This evaluation is an independent, trauma-informed system designed to help you clearly identify and document the full scope of your service-connected mental-health conditions, including PTSD, MST, depression, anxiety, and related impairments. It serves to support the need for appropriate clinical treatment and, when appropriate, strengthen the accuracy and completeness of your VA disability claim.

Two primary goals guide this process:

  1. Clinical Goal #1   Evaluation of Need
    To determine the extent to which untreated or undertreated service-related mental health conditions are interfering with your daily functioning.

  2. Clinical Goal #2   Connection to Care
    To help you identify and access appropriate VA and or community-based treatment resources you have earned through your service.

Why This System Exists

Many veterans have been ignored, misdirected, or discouraged by the very institutions charged with helping them.
Too often:

  • Clerical staff or unlicensed screeners have blocked access to VA psychiatrists, misclassifying mental health needs.

  • VA clinicians have stated they “cannot advocate” for veterans during the Compensation & Pension (C&P) process, leaving veterans unrepresented.

  • Spouses and loved ones have been barred from participating in evaluations, despite their crucial role in providing accurate information.

  • Veterans with legitimate trauma histories have been told their records were “lost in the 1973 fire” or they are not eligible, causing years of delay.

Our MH-PSA system directly confronts these barriers. We do not accept indifference, delay, or disrespect toward those who served. Our approach blends clinical rigor with veteran advocacy, ensuring each participant receives an accurate, fair, and complete assessment.

On Point 4 Veterans:

If you feel overwhelmed, take a break or invite someone you trust to join you.
Remember—this process is designed to protect you and to make sure your voice is heard.”

Guides (examples):

  • What if I can’t remember exact dates or units? → “Provide your best recollection; context is often enough for verification.”

  • Can my spouse help me answer? → “Yes, and it is strongly encouraged.”

  • What if my records were destroyed or lost? → “We can help you document alternate proof of service or exposure.

Is this connected to the VA or Vet Center? → “No. This assessment is independent and solely for your use and review with your clinician.”

Your Role and Responsibilities

  • Be truthful and thorough. Your success depends on accurate information linking your current symptoms to in-service events.

  • Participate actively. Our website and podcasts are your AI guide through structured prompts, pauses, and clarifications as needed.

  • Involve a support person. We strongly recommend completing this assessment with a spouse, family member, or trusted friend present.

  • Provide documentation. You are responsible for locating any service or medical records that substantiate your experiences. If you cannot find them, we may assist you, at your request, in locating the records required to validate your claims.

  • Review your results. After completion, you can choose to meet with the clinician to discuss findings and next steps.

Remember, your Avoidance and Denial harm you and your family.  Speak up.

SUPPORT FROM OP4V IS A FLAT FEE OF $600, YOU WILL RECEIVE

The comprehensive MH-PSA evaluation, including intake, clinician review, diagnostic scoring (PCL-5, PHQ-9, GAD-7), summary preparation, and ongoing evaluation/consultation.

You will receive a full report and an opportunity to ask questions before finalization.
